Transitional Housing Programs

What We Offer

At Children’s Hope Foster Family Agency, we provide transitional housing programs for young adults ages 18–25 who are exiting or have exited foster care. We understand that transitioning into adulthood comes with challenges—and we’re here to support each youth every step of the way.


We offer two distinct programs designed to provide housing, mentorship, and life skills training:


  • Transitional Housing Placement – Plus Foster Care (THP+FC)
  • Transitional Housing Program – Plus (THP-Plus)

Program 1: THP+FC (Transitional Housing Placement – Plus Foster Care)

Ages Served: 18–21
Eligibility: Non-Minor Dependents still under foster care, including youth on probation.

This extended foster care program offers safe, supportive housing and access to comprehensive services. Each participant receives a Transitional Independent Living Plan (TILP) from a county social worker and is supported by a Children’s Hope social worker.



Program Focus:

  • Personalized case management
  • Mental health and life skills support
  • College/trade school or employment requirements
  • Assistance in meeting independence goals
Note: Youth in guardianships who are not dependents are not eligible.

Program 2: THP-Plus (Transitional Housing Program – Plus)

Ages Served: 18–24 (up to 25 where extended)
Eligibility: Youth who exited foster care on or after their 18th birthday

THP-Plus provides a supportive living environment for young adults who have exited the foster care system. Participants receive coaching, accountability, and encouragement to build a foundation for self-sufficiency.

  • Career and financial guidance
  • Ongoing life skills development
  • Emotional and wellness support

Our Approach

Each youth is assigned a Children’s Hope social worker who assists with:

  • Goal setting
  • Financial planning
  • Academic support
  • Job readiness and placement
  • Access to mental health services

Our mission is to create a bridge to adulthood where youth feel safe, supported, and empowered.

Eligibility & Requirements

To participate in either program, youth must:

  • Be enrolled in college, trade school, or employed
  • Participate in case management and life skills services
  • Commit to their individualized housing and independence plan
